Brian Juul, Community Liaison
Brian was born in St. Louis Park and graduated from Richfield High School. He received his Associates Degree in Sales Marketing from Normandale Community College, and later earned his Bachelor’s Degree in Business Administration with a minor in Sales Marketing from Mount Marty College, SD. His training and philosophy are grounded in an integrity-based approach to his work—going the extra mile to solve needs in an honest and respectful manner.
Brian's work at Home Health Care, Inc. includes a dual role of Director of Sales with Home Health Care, Inc. and Director of Business Development for Legacy Home Care, our private pay brand. He enjoys cultivating relationships with referral sources and educating them on the different lines of service we offer. A favorite part of his job is meeting with families and seniors who are experiencing a health care crisis—helping them navigate through it by providing resources, guidance, and education on all viable options.
Brian is currently a member of the Minneapolis and St. Paul Area Senior Workers Associations, as well as the Alzheimer's Association. When not in the office, he enjoys working out, walking area lakes, jet skiing, roller blading, tennis, and riding his bike. Brian is also an avid car buff and has a dealer's license. He enjoys attending auto auctions.

Kim Wagner, LSW, Community Liaison
Kim was born and raised in Apple Valley. She attended Minnesota State University, Mankato and earned her Bachelor of Science in Social Work (BSSW). As a Licensed Social Worker, Kim has worked in a variety of settings including transitional care, long term care, and memory care units. Kim is an active member of the National Association of Social Workers (NASW), the St. Paul Senior Workers' Association, as well as the President of the MNHSWA (Minnesota Nursing Home Social Workers’ Association.)
As a community liaison for Home Health Care, Inc., Kim is available to meet with home care clients, family members, and referral sources to provide personalized services. Her approach is always client centered. One of Kim's philosophies is that attitude is a choice. We have a choice each day regarding the attitude we will embrace for that day—and for Kim, a positive attitude is key.
When not in the office, Kim enjoys spending time with family and friends at their cabin in Wisconsin. Kim also enjoys playing cribbage, going to movies, and trying new restaurants. Kim is a loyal Twins and Vikings fan and supports Minnesota teams by consistently watching and attending the games. Kim also takes pleasure in cooking; trying new recipes and attending classes.

Karissa Borchert, MSW, LGSW, Community Liaison
Karissa is originally from a small town in southwestern Minnesota and currently resides in Plymouth, MN. She graduated from Minnesota State University, Mankato with a Bachelor of Science in Psychology and completed her Masters of Social Work (MSW) from the University of Minnesota.
Karissa has worked in a variety of social service and medical settings most frequently with adults and elders with mental illnesses and neuromuscular diseases. She is a Qualified Developmental Disability Professional (QDDP), and has received her LGSW.
Karissa has a passion for helping clients find and utilize needed services in an effort to have them maintain a healthy, stable lifestyle. She understands that each client has unique needs and works to ensure they receive the best care and services possible. Karissa enjoys working with a client's entire team, including family members and referral sources to make a transition as smooth as possible.
When not in the office, Karissa keeps busy with her little boy, training for and running marathons, golfing, traveling, and going to concerts. Karissa enjoys spending time with family and friends—and exploring new attractions and restaurants in the metro area.

Dana Bergerson, COTA/L, Clinical Liaison
Dana was born in Bremerton, WA and raised in the Chisago Lakes area of Minnesota. She graduated from Chisago Lakes High School and received an Associates of Applied Science degree as a Certified Occupational Therapy Assistant and a Bachelor’s degree with a minor in Gerontology.
Prior to joining the Home Health Care, Inc. team, Dana worked in a myriad of different healthcare-related positions in the areas of nursing, therapy, aquatic rehabilitation, home care, and hospice.
Having experienced first-hand the reality of caring for elderly family members, Dana is passionate about helping individuals handle the challenges of growing older with dignity and confidence. She loves educating geriatric patients and families on their care options and empowering them to be stronger advocates for themselves.
When not in the office, Dana resides in the Chisago Lakes area with her husband and three children. She is an active member of her church, singing regularly in services. She also loves spending time with friends and family at the cabin, exploring the North Shore, scrapbooking, and escaping to warmer climates with her family when she needs a break from the Minnesota winter.

Jessica Melcher, MSW, LISW, Clinical Liaison
Jess was born and raised in Edina, MN, in a neighborhood with very few children and lots of elders. So she was the neighborhood helper for the seniors – cleaning, running errands, and providing companionship.
After graduating from Edina High School she completed her B.A. in Psychology from the University of Minnesota. She obtained her Masters in Social Work from the University of Minnesota, with a focus on geriatrics that included two internships in senior services.
Her 12 years in social work have included the following institutional settings – in a hospital/clinic system as a patient advocate, as a mental health worker in a psychiatric rehab facility, and as a social worker in long term care facilities and a transitional care unit. Her experience in these settings have included teaching independent living skills and doing community case management with disabled adults.
In her current role as a home care social worker, Jess loves meeting with clients in their homes, identifying unmet needs, and helping them navigating the many healthcare support systems.
An animal lover, Jess has two rambunctious kitty cats and hopes to have dogs again someday, too. In her free time, Jess likes running around our many area lakes and doing yoga and barre. She also enjoys reading historical fiction and poetry.

Megan Hickenbottom, MSW, LGSW, Clinical Liaison
Megan was born and raised in Plymouth, MN where she graduated from Wayzata High School. She received her undergraduate degree in Social Work from Minnesota State University, Mankato and obtained her Masters of Social Work degree from the University of Minnesota.
Megan has worked in healthcare for the past six years. Her roles have varied—from providing direct client support to managerial positions within the healthcare field. Most recently, she worked with adults who are enrolled in county waiver programs.
The best part about her job so far is the fast-paced environment and variety of work. Megan also really appreciates working for an organization whose mission is to provide the best care to each individual we serve.
Outside of work, Megan enjoys spending time with her family, including her boyfriend, their daughter, and two dogs. She enjoys reading, long bike rides, and being outdoors.

Elizabeth Gionfriddo (Liz), COTA/L, Clinical Liaison
Liz was born and raised in Shakopee, MN. She and her older brother enjoyed spending time outside on their parent’s five acre property—with apple trees and a large garden.
Liz graduated from the College of St. Catherine with an Associates in Applied Science (AAS) in Occupational Therapy (OT). She also has her PAMS (Physical Agent Modality) certification. This includes procedures and interventions such as the use of hot packs, cold packs, ultrasound, electrical nerve stimulation and more. Liz also has her CarFit certification through AARP, ensuring that older adults have cars that “fit” them best for maximum comfort and safety. She is currently studying for her BA in Healthcare Management with a graduation date slated for May 2018.
Liz began her career as a CNA/PCA before spending several years as an OT in home care. She has three children ranging in age from 8 to 14, as well as a 16-year old dog, Daisy, and two geckos!
When not at work she likes being active, including playing volleyball, jogging, hiking, taking walks around the lake, rollerblading, canoeing, kayaking—and participating in her children’s activities (or being a spectator). On Sundays she enjoys sharing a meal with her family—including her grandmother, aunt, sister-in-law, nieces, nephews, parents, brother and kids.
