Physical Therapy
Physical Therapy services may be needed after a hospital stay, a fall, or simply due to loss of function from normal aging. Our physical therapists work to help you get through the rehabilitation process and back on track. Physical therapy services may include balance exercises, stretching and light muscle movement to help improve health and prevent future injury.
Physical therapy helps to:
- Decrease pain and stimulate healing
- Reinforce function and mobility
- Encourage further health and fitness education
- Conduct environmental and safety checks to enhance your independence
Occupational Therapy
Occupational Therapy (OT) assists individuals of all ages perform activities of daily living (ADLs). Our main goal is to allow you to function well in your home. Our occupational therapists can assist with tasks like cooking, dressing and other things you may have done prior to an injury or illness. Occupational Therapists also help with equipment use (e.g., Walkers, wheelchairs or other adaptive devices that enable you to continue with your independence.)
Our occupational therapists can:
- Evaluate and train clients for daily living activities
- Teach exercise programs to increase joint motion, strength, and flexibility
- Instruct you about joint protection and personal energy conservation
- Make recommendations to increase home safety
- Train in the use of adaptive devices
Speech Therapy
Speech Therapy may be needed as a result of trauma, stroke, or other medical conditions. Our Speech-Language Pathologists (SLPs) help you regain and increase your ability to communicate through speech. They can also addresses problems with quiet or slower speech, slurring, and swallowing. Speech Therapy can help you improve your verbal articulation and develop better movement of the lips, tongue, and jaw.
Our speech therapists focus on:
- Recognizing communication issues and their ability to be improved
- Setting measurable goals with the client and their family
- Treating voice disorders and dysphagia (difficulty swallowing)
- Educating you and your family about speech irregularities
- Promoting healthy lifestyle practices that enhance your speech
Our specially trained Home Health Aides (HHAs) provide a variety of hands-on services that can help you with:
- Bathing, washing hair and shaving
- Dressing and toileting
- Feeding and meal preparation
- Mobility and assisting with range-of-motion exercises
In addition to light housekeeping and personal care, our home health aides:
- Perform delegated tasks supervised by a registered nurse
- Document care and observations, notifying the registered nurse of any health changes
- Follow and assist you with your plan of care
Our Social Workers use their years of experience to help clients and families find solutions in the sometimes confusing world of healthcare. They specialize in:
- Finding resources for necessary care
- Coordinating meal delivery and other necessary services
- Advising on housing, transportation, and long-term care options
- Helping with outreach services, crisis prevention, and social rehabilitation
- Planning supportive services for your return home
- Assisting you, your caregivers, and other homecare staff
